Page 4-line 10                                        After "occurred" insert ".  If the commission determines that an error has been made, it shall inform the injured individual or his representative,"; after "and" insert "then".

Page 4-line 12                                        After "law" delete "and" insert ".".

Page 6-line 6                                          Delete "to" insert "grant immunity from civil liability to anyone who reports an error.  Upon application by an injured party, the commission rather than a court of law would determine the amount of damages recovered as a result of any injury or death caused by a medical error.  All heath care providers would be required to pay a premium to the commission in exchange for immunity from civil liability.".
