TPACT Consent Forms
As part of the evaluation of the Back to School Program, TPACT must obtain permission from parents for their children to participate, as part of the conditions of the federal grant to conduct the study. TPACT would also like specific consent from either the building principal or the district superintendent for the teacher to participate in the evaluation. The consent forms in .pdf files are available for teachers to download on the left-hand side of this page.
The parental consent form should be sent home in advance of the date the students are taking the pre-test. The first page is for the parent to keep and the second page should be returned to the school. The form contains check boxes for granting or withholding permission to participate and a parent’s signature line. If a parent does not want the student to participate, he or she should check the “No” box and sign the form. It is important for the parent to sign and return the form, even if they do not want their child to participate in the study.
